The first half didn't record any big talking points for either side, but Khuliso Mudau slotted home Aubry Modiba's long-range spilt freekick to give the visitors the lead on 67 minutes.
However, the hosts levelled matters after Sundowns goalkeeper Ronwen Williams poorly controlled a back pass allowing Al Hilal captain Mohamed Abdelrahman Yousif Yagoub to pounce and score the equalizer four minutes after Sundowns had taken the lead.
The game had one more dramatic moment left; in the 90th minute, Sundowns' Kenyan international Brian Onyango fouled Al Hilal forward Yasir Mozamil Mohamed El Tayeb in the forbidden area, and a penalty was awarded.
Up stepped Ather El Tahir, but Ronwen Williams saved from 12 yards to deny the hosts an opportunity to confirm a knockout stage spot for now.
A win for Florent Ibenge's charges would have sealed a quarter-final spot, but the draw now means Al Hilal must win in Cairo in the final group match day to advance.
Table-topping Masandawana now move to 11 points, one ahead of second-placed Al Hilal and four ahead of third-placed Al Ahly.
Rhulani Mokwena's charges host Coton Sport in their final group game.
